Responsibilities

Provides clinical pharmacy services including but not limited to assessment, monitoring and optimization of patient specific pharmaceutical care. Serves as a departmental and organizational resource in area(s) of expertise and provides educational programming and staff development to physicians, pharmacists, nurses and other practitioners in assigned specialty area. Serves all patient populations including neonates, pediatrics, adolescents, adults and geriatrics.

 

Transition of Care MTM Clinical Pharmacist

 

  1. Performs duties of a clinical pharmacist in providing pharmaceutical care to patients with complicated medication regimens who are at risk for readmission.
  2. Monitors all aspects of medication use related to patients in defined populations at risk for readmission.
  3. Conducts post-discharge phone assessments for patients discharged from Transition of Care Program.
  4. Develops and maintains effective working relationships with key physicians, mid-level providers, case managers and nursing personnel in service area.
  5. Participates in quality initiatives including evaluation and analysis of medication use practices.
  6. Assists in development of medication use protocols specific to area of practice.

Qualifications

REQUIRED:

  • Graduate of an accredited School of Pharmacy.
  • Doctor of Pharmacy, OR Master's Degree with completion of a residency in the assigned clinical area or equivalent experience.
  • Two (2) years previous hospital pharmacy experience.

 

PREFERRED:

  • Completion of an accredited post-graduate year 1 (PGY-1) residency 
  • Certification by Board of Pharmacy Specialties (BPS) is strongly encouraged in the clinical practice specialty – Board Certified Pharmacotherapy Specialist (BCPS), Board Certified Nutrition Support Pharmacist (BCNSP), Board Certified Oncology Pharmacist (BCOP), etc.
