Pulmonary & Critical Care of Atlanta’s board-certified physicians provide the highest quality of care to metro Atlanta patients with pulmonary diseases and critical care needs.

 

Because we are a Northside Network Provider, our physicians have access to advanced technologies through the Northside Hospital Pulmonary Therapeutic & Diagnostic Endoscopy Lab.

Responsibilities

The Nurse Navigator is responsible for the coordination of patient care across the continuum under the auspices of a provider’s prescribed plan of care, national guidelines, and within the scope of nursing practice.  The navigator educates/provides information and support to patients in order to guide and facilitate understanding of treatment plans prescribed by licensed practitioners and/or within scope of nursing practice.  Monitors patient outcomes and participates in quality improvement activities.  Contributes to and collaborates with health care team members to positively impact patient outcomes and patient experiences.  Facilitates a clear line of communication between referring physicians and care providers to avoid delayed treatment planning, missing opportunity for early diagnosis and treatment of potential cardio toxicity.  Follows-up with referring physicians, providing updates on the patients and families referred to the program.

Qualifications

KNOWLEDGE SKILLS AND ABILITIES/LICENSE OR CERTIFICATION REQUIRED

  • Bachelor of Science Degree in Nursing and Bachelor of Science Degree in Nursing and three (3) years of clinical experience in Cardiology OR Associate Degree in Nursing with five (5) years of clinical experience in Cardiology
  • Registered Nurse license in the state of Georgia
  • Working knowledge of service line and strong foundation in patient support programs
  • Establishing and maintaining effective working relationships with patients, medical staff, and the public. Ability to react calmly and effectively in emergency situations.

  • Strong interpersonal skills
