Responsibilities

Provides clinical pharmacy services including but not limited to assessment, monitoring and optimization of patient specific pharmaceutical care. Serves as a departmental and organizational resource in area(s) of expertise and provides educational programming and staff development to physicians, pharmacists, nurses and other practitioners in assigned specialty area. Serves all patient populations including neonates, pediatrics, adolescents, adults and geriatrics.

 

Oncology Pharmacy Clinician

  • Performs duties of a clinical pharmacist in providing pharmaceutical care to Oncology patients.
  • Monitors all aspects of medication use in the Oncology population.
  • Applies the pharmaceutical care process to care for and treat oncology and hematology patients.
  • Prepares and/or checks preparation of chemotherapy and applies chemotherapy safety program to chemotherapeutic medications.
  • Develops and maintains effective working relationships with key physicians and nursing personnel in service area.
  • Participates in quality initiatives including evaluation and analysis of medication use practices.
  • Assists in development of medication use protocols specific to area of practice.

Qualifications

REQUIRED:

1. Graduate of an accredited School of Pharmacy.

2. Doctor of Pharmacy, OR

a. Master's Degree with completion of a residency in the assigned clinical area or equivalent experience.

3. Two (2) years previous hospital pharmacy experience.

 

PREFERRED:

Certification by Board of Pharmacy Specialties (BPS) is strongly encouraged in the clinical practice specialty - Board Certified Pharmacotherapy Specialist (BCPS), Board Certified Nutrition Support Pharmacist (BCNSP), Board Certified Oncology Pharmacist (BCOP), etc.
