The Care Coordination Trainer will ensure competency and proficiency by providing the tools and training to support the hospital’s mission of driving high-quality utilization of healthcare resources. The Care Coordination Trainer will help shape the form of a Training program for the Care Coordination Department that will include the UR and Discharge Planning functions. The Trainer will maintain the processes and education for the necessary Care Coordination interventions to include patient assessments, clinical reviews and knowledge of community resources among others, with the goal of assuring effective and efficient clinical resource management under the direction of a Care Coordinator Manager or Director.
KNOWLEDGE SKILLS AND ABILITIES/LICENSE OR CERTIFICATION REQUIRED Graduate of an accredited Registered Nursing School (RN) with a current GA RN license or MSW/BS degree in related field with strong clinical background and or related experience. Two or more years’ experience in Medicare, Commercial and Medicaid medical UR review Two or more years’ experience with hospital discharge planning Excellent oral and written communication skills. Proficient user of Microsoft office suite such as Outlook, Access, Excel and Word Ability to travel between campuses as needed KNOWLEDGE SKILLS AND ABILITIES/LICENSE OR CERTIFICATION PREFERRED LICENSE OR CERTIFICATION REQUIRED BY THE STATE OF GEORGIA Current GA RN License
